Required by a procedure, in nobody's job description

Anyone who keeps a management system knows this. Procedures get revised, job descriptions don't, and soon nobody knows who owns what. It surfaces in the internal audit, with the report to be written from transcripts and notes before the certification body comes.

One case on our Try page: job descriptions and procedures, plus an internal audit's transcripts and notes. Yesper built the RACI chart and the audit report. Ten of the 45 activities had unclear or missing ownership, and the job hazard analysis required before field work was in no job description. Major nonconformity.

The ownership gap shows in the chart before the audit, and the report lands with every finding tied to its evidence, for the auditor to confirm. Same for the neighbouring jobs on the Try page: a job hazard analysis for a refrigeration install, a portfolio review before the PM meeting.
