Training and certification register, SR 907 Spruce Hollow Overcrossing

Credentials for 100 workers from seven companies, read from 231 training records and photographed cards. The register shows what has expired or expires within 90 days, what is missing for each task, and which file every entry comes from.

Summary

Scheduled high-risk tasks

4 workers are scheduled for high-risk tasks without a current credential

The lookahead puts David Smith on the falsework beam pick Nov 17 with an NCCCO certification that expired Oct 21, and Brittany Scott on rebar placement with a telehandler evaluation that lapsed Sep 24. Nathan Williams is scheduled inside the track zone Nov 21 with a railroad orientation that expired Aug 21. No fall protection record was found for Luis Davis, who is on the north deck edge forms.

To confirm: the safety manager should check the subcontractors’ original records and reassign or hold these crews before the work starts.

Corbin Traffic Control

8 of 10 flagger cards expire Dec 2

Seven flaggers and the lead flagger took the same class on Dec 2, 2023. A Washington flagger card expires no later than 3 years after issue, so all eight lapse Dec 2, 2026, six days before the night lane closures for the girder set Dec 8–9. A recertification class needs to be booked before then.

To confirm: the safety manager should verify with Corbin that a recertification class is booked before Dec 2 and that no one flags the closures on a lapsed card.

Card photos

9 entries from photos could not be read reliably

On 9 of 126 gate-check photos, glare, blur or worn cards left the date uncertain (confidence below 0.90). Two of them belong to workers on the girder set crew. The entries are flagged for a check against the original cards.

To confirm: the safety manager should check the nine entries against the original cards, starting with the two on the girder set crew.

    Method

    1. The sign-in roster gave the worker list. Each record and photo was matched to a worker by name, company and the last four digits of the badge number.
    2. Text-searchable pages were read directly. Scanned pages and photos were read with OCR and image recognition. Each entry was stored with its file, page or photo name and a read confidence between 0 and 1.
    3. Expiration dates were taken from the card or certificate where printed. Otherwise they were calculated from the completion date and the rule for that credential, shown in the detail panel.
    4. Requirements per worker follow from the tasks assigned in SSSP Appendix B. Status is as of Nov 6, 2026: expired, expires within 30, 60 or 90 days, current, or missing when a requirement has no record.
    5. Lookahead activities were checked against the requirements on the last day of each activity.
    6. Entries with read confidence below 0.90 were flagged for manual review.

    Assumptions and limitations

    • Entries with read confidence below 0.90 are not confirmed. The safety manager should check them against the original card or certificate. Other readings from photos were not checked by hand.
    • Task requirements come from SSSP Rev. 3, Appendix B. The safety manager or the competent person for each task should confirm them against the current site-specific safety plan and the railroad’s contractor requirements before the register is used for crew assignments.
    • OSHA Outreach cards do not expire; the 5-year limit is the SSSP’s. Washington rules set no fixed interval for aerial lift or fall protection training; the 3-year limits are SSSP requirements. Confined space training has no expiration and is shown with ✓ when a record exists.
    • Crane operator hours, substance abuse tests, employer evaluations and other employer authorizations are not in the records and are not checked here.
    • Certificates were not verified with the issuers, for example through NCCCO’s online verification.
    • A course record shows completed training, not a designation: competent persons, NFPA 70E qualified persons and equipment operators are designated or authorized by the employer. Missing means no record was found in the files, not that the training never took place.
    • A badge number links a record to a worker on the sign-in roster; it says nothing about current employment.
