Remediation Completion Report, 418 W. Main Street, Granite Run, Indiana
The 41 project files from the cleanup of the former service station on SR 466 are indexed, sorted into revision chains and placed on a timeline. The completion report is drafted in the RCR template with a source for every section. Three items need to be resolved before it goes to IDEM.

Summary
Revision chain · WRA 2606118
Grid C4 was cleared on a superseded lab report
Rev 1, issued Jun 19, corrects transposed sample IDs C4-F and C4-SW. The C4 floor has 3.5 mg/kg benzo(a)pyrene against a cleanup level of 2.1 mg/kg. Phase 2 was backfilled Jun 22 on Rev 0 data, and Progress Report No. 2 and summary Rev B still carry the wrong value.
Action: correct the result with IDEM and decide on re-excavating the C4 floor.
Agency correspondence · VRP No. 6260412
IDEM comment 3 was never addressed
The Jul 8 IDEM letter has five comments. The Jul 29 response covers 1, 2, 4 and 5, but not comment 3 on how the stockpile at 422 W. Main Street is protected from runoff. A Jul 2 email shows soil was stockpiled there.
Action: respond to IDEM before the report is submitted.
SAP Rev 2 · Dewatering
Three weeks without discharge samples
The approval letter and the SAP require weekly samples while dewatering. There are no lab reports for the weeks of Jul 13, Jul 20 and Jul 27, but the flow log shows 486,000 gal discharged to the sanitary sewer.
Action: report as a deviation in Sections 6 and 8.
The file record, file by file
The timeline shows agency correspondence, field work, dewatering and document revisions. The file index has a status for all 41 files, the revision chains show which revision is used where, and the completion report traces each section to a source file and page.

Method
All 41 files were given a number, category, date, revision and status. Scanned documents (IDEM site visit memo and disposal certificates) were read with OCR.
Documents with a common origin were linked into revision chains using document numbers, revision blocks and supersedes statements.
Dates from letters, approvals, minutes and logs were placed on the timeline. Each IDEM comment was matched to the response, comment by comment.
Every result in progress reports and summary tables was checked against the current revision of the lab report.
Discharge sampling was compared week by week with the flow log and with the approval conditions and the SAP.
The completion report was written in the ten sections of the RCR template, with a file and page reference for each statement.
Assumptions and limitations
Current means the latest revision in the project folder. Documents held only by IDEM or in someone's email may be missing.
Comment 3 being open refers to these 41 files. A verbal response during the Aug 18 site visit is not recorded in the IDEM memo.
One sample per grid does not delineate how much of Grid C4 exceeds the cleanup level.
The completion report is a draft. Sections 3, 5, 6, 8 and 9 need to be completed before it is submitted to IDEM.
“RWP Addendum No. 1” is the name used in the project file. IDEM’s published VRP forms do not use that title, so the IDEM project manager should confirm how a change to the approved RWP is to be submitted.
The discharge limits for benzene, oil and grease and TSS are taken from temporary discharge authorization TDA-26-07 as written. Whether they match the utility’s pretreatment requirements should be confirmed with the utility’s pretreatment coordinator.
