Summary

RACI · Procedure only

A job hazard analysis before field work is required by PR-07 but missing from all 11 job descriptions

PR-07 Sec. 4.5 (p. 39) assigns the JHA to the Project Manager, with Department Manager approval. None of the four sampled site visits had a JHA (auditor’s notes, p. 5). Major nonconformity IA-03-01.

RACI · Multiple A

Two roles are accountable for closing out QC comments, and in practice neither does

JD-05 Principal-in-Charge and JD-06 Project Manager (both Sec. 3.8) claim it. In interviews 2 and 3 each points to the other, and two of three sampled releases went out with open comments (7 and 3). Minor nonconformity IA-03-02.

Audit report · Conflicting evidence

The notes call the management review complete; the President says the EHS part was deferred

The auditor’s notes (p. 2) record all inputs covered on Mar 12, 2026. In interview 1 (00:41:20) the President says environmental and OH&S items moved to November. IA-03-05 stays unclassified until the minutes are checked.

RACI chart and audit report

The chart shows who is Accountable (A), Responsible (R), Consulted (C) and Informed (I) according to the job descriptions. Dashed cells are assignments that exist only in a procedure. Click a row for its sources and related findings, or a role code to highlight that role. In the report, each finding links to its evidence and back to the chart.

Method

  1. Responsibilities from the job descriptions. Each statement in Section 3 was matched to an activity in the procedures. “Accountable for” and “responsible for ensuring” were read as A; “performs,” “prepares,” “conducts” and similar as R; “takes part in” and “comments on” as C; and “is informed of” and “receives” as I. Where a role both owns and performs the work, it is shown as A/R.
  2. Chart checks. Each activity should have exactly one A. Role assignments in the procedures that are missing from the matching job description are flagged “Procedure only.”
  3. Audit report. The transcripts and notes were read against the audit plan’s questions. Statements and sampling results were linked to activities in the chart and to clauses of ISO 9001:2015, ISO 14001:2015 and ISO 45001:2018, the editions the firm is certified to. ISO 14001:2026 (published Apr 15, 2026) and ISO 9001:2026 (published Sep 16, 2026) have replaced the 2015 editions, but certificates to them remain valid during the transition period.
  4. Grading. Findings were graded using the audit plan’s definitions. Where the notes and the transcript disagree, the finding is left unclassified and flagged for verification.

Assumptions and limitations

  • Ownership is taken from the job description text. How the work is actually split shows only where the interviews or sampling cover the activity.
  • The transcripts are automatic. Quotes are reproduced as transcribed, with time stamps.
  • Grading is a proposal; the auditor issues the report.
  • The audit covered the Quail Grove office and a sample of three projects. Activities outside the sample have no findings.
  • The cross-reference gives clause numbers and headings only, not requirement text.
