Work plan for the SR 389 culvert replacement at Kettle Grove Creek
Work plan WP-389-03 for replacing the culvert under SR 389 at Kettle Grove Creek, written on the company's form WP-01 (Work Plan and JHA). The 11 special conditions in the Corps' Nationwide Permit 14 verification are tied to the 11 work activities. Three conflicts between the documents need to be resolved before work starts.

Summary
Plan conflict
Culvert invert differs 0.50 ft between the profile and the typical section
Profile Sheet C-302 Rev A sets the invert 0.5 ft below the natural streambed; typical section Sheet C-501 Rev B sets it 1.0 ft below. Special Condition 5 of the verification (p. 3) requires at least 1.0 ft, so only the typical section meets the permit.
The work plan uses 1,411.10 at centerline, and RFI-07 goes to the Engineer.
To confirm: the Savannah District regional conditions call for 20% of the culvert height, 1.1 ft for this pipe. The Corps should confirm which depth governs before bedding.
Work window
6 in-stream workdays fall after October 14
Baseline Schedule Rev C puts part of the pipe installation, the streambed material and the diversion removal after the work window. Special Condition 2 (p. 2) prohibits in-stream work from October 15 through March 31.
Starting the whole schedule three weeks earlier clears the window.
Staging
Fuel tank and spoil stockpile are too close to the creek
The Staging and Laydown Plan puts the fuel tank 60 ft and the spoil stockpile 20 ft from the creek. Special Condition 7 requires 100 ft for staging and fuel, and Special Condition 8 requires 50 ft for stockpiles (p. 3).
The work plan adds a hold point for a revised staging plan before mobilization.

Method
The special conditions in the verification letter were split into 11 checkable requirements with page references. The NWP general conditions attached to the letter were read for context and are referred to by name only.
The work activities were taken from the special provisions and Baseline Schedule Rev C and entered on form WP-01 with work steps, hazards, controls and hold points.
Each condition was tied to the activities it governs, and each activity carries the condition's exact wording.
Invert elevations at the inlet, centerline and outlet were compared between the profile sheet and the typical section, and against Special Condition 5.
Dates of the in-stream activities were checked against the work window in Special Condition 2. Workdays are counted Monday–Friday.
Distances on the staging plan were measured from the top of bank and compared with Special Conditions 7 and 8.
Assumptions and limitations
Elevations are NAVD 88; coordinates are NAD83(2011) State Plane, Georgia West zone, in US survey feet. The natural streambed is the surveyed line on the profile sheet.
Activities 4–7, 9 and 10 count as in-stream work. Backfill (Activity 8) is placed over the installed pipe and is not counted; if the Corps views it otherwise, more workdays fall outside the window.
Holidays and weather days are not counted in the workday totals.
Hazards and controls follow form WP-01 and OSHA 29 CFR Part 1926. The competent person must review the excavation and lifting steps against the geotechnical report and the lift plan before the work plan is used.
The traffic control plan, the erosion, sedimentation and pollution control plan (ES&PC plan) and Georgia stream buffer requirements were not reviewed here.
The Savannah District's 2026 NWP regional conditions ask for culverts in perennial streams to be embedded 20% of the culvert height, which is 1.1 ft for a 66-in pipe. Special Condition 5 and typical section Rev B use 1.0 ft. Confirm with the Corps which depth governs before bedding (RFI-07).
The verification is issued under NWP 14 (Linear Transportation Projects). Replacing an existing culvert could also be authorized under NWP 3 (Maintenance); the Corps project manager should confirm which permit applies.
The October 15 – March 31 window is taken from Special Condition 2. The Savannah District regional conditions contain no such period, and it may come from a state or Section 401 condition. Its dates should be confirmed with the Corps and Georgia DNR before the schedule is changed.
